Client Portal – Current Clients Only

The Client Portal is for current/active therapy clients and for clients whose licensed healthcare provider has already submitted the referral form for an autism evaluation for their child.

If you’re interested in an autism evaluation for your child and have not yet had your child’s provider submit a referral form, please start by having the provider complete the online form or download the referral form and fax (toll-free) to: 505-212-4412. After the referral form has been received, we will contact you with the next steps (complete intake form etc.) required before scheduling an evaluation.

Once your initial paperwork (referral and intake forms, etc.) has been processed, you will receive an email from “app@intakeq.com” (subject: Your Messaging Portal with Michele Iemolo) inviting you to the portal. You will be prompted to click the green tab in the email that says, “Continue to Portal”. This button will then take you to the registration page, where you can create your account/login password. After this step is complete, you will receive a confirmation email with the option to sign-in to your account. You are now considered as an active/current client. You can return to that email, bookmark the portal in your browser or use the button below to access the portal by entering your password.

As an active/current client, you’ll use the Client Portal to access and complete the remaining forms needed for evaluation, upload any necessary documents, check your appointments, and/or retrieve your child’s comprehensive report when ready.
