Embodied Heart Space, LLC provides Comprehensive Diagnostic Evaluations (CDE) for children ages 1.5 to 4.11 years old only, with developmental differences and concerns of possible autism spectrum disorder (ASD). Dr. Iemolo does not see children older than 4.11 years old. Autism Evaluations are conducted in English only and referrals are on a first-come, first-served basis.

All evaluations require a completed referral from a licensed medical professional (e.g., PCP, psychologist, NP, etc.)

For clients with Medicaid (Presbyterian or BCBS) the referral MUST come from a medical professional who has an individual Medicaid Number. The Medicaid enrollment must belong to the individual practitioner (Type 1 NPI) rather than a generalized group or facility number.

The Autism Evaluation Process

Step 1:

Medical Provider Referral

If you are concerned that your child may be showing signs of autism spectrum disorder (ASD) and are interested in an evaluation through Embodied Heart Space LLC, your first step to is ask your child’s current licensed healthcare provider (e.g., pediatrician, CNP, PCP, psychologist, speech-language pathologist or early intervention (EI) provider) to complete the referral form. The licensed healthcare provider filling out the referral form should be currently treating your child and whose scope of practice allows them to perform an autism screening and document concerns that warrant further evaluation. 

If you have Medicaid (Presbyterian or BCBS), the referral MUST come from a medical professional who has an individual Medicaid Number (Type 1 NPI) rather than a generalized facility number (Type 2 NPI).

 Step 2:
Intake and Scheduling

Once the Referral form is received, we will email you with the steps required before scheduling an evaluation. These include completing an intake form, rating scales, and insurance paperwork via our secure online Client Portal. Once we have received and reviewed your intake packet, we will contact you to either schedule an evaluation or discuss clinically indicated referrals/ recommendations to other providers.

We strongly encourage you to begin gathering your child’s medical and educational records, including copies of reports if your child has received testing or services through Early Intervention, Child Find, school records, including Individualized Education Programs (IEPs), Applied Behavior Analysis, Speech and Language, or Occupational Therapy evaluations. These records will be requested as part of your child’s evaluation process once your intake paperwork is received and reviewed.

What to Expect During the Evaluation Process

At Embodied Heart Space we know that “doctor visits” or going somewhere new can sometimes be challenging for young children with sensory differences or difficulty with changes in routine. During your evaluation appointment, Dr. Michele works at your child’s pace, giving them the time and space they need to become comfortable with the environment in their own way.

As a parent or caregiver, you’re a crucial part of your child’s evaluation. We’ll ask about your child’s development and have you complete additional rating scales as needed. Depending on how old your child is, you may also participate in a play-based assessment with your child and the psychologist. Our comprehensive ASD evaluation assesses developmental, cognitive, adaptive, behavioral, and social-communication skills. The ADOS-2 (Autism Diagnostic Observation Schedule, Second Edition) is a standard part of every in-person evaluation at Embodied Heart Space.

Following your child’s evaluation, we’ll provide feedback along with a comprehensive report containing treatment recommendations. Expect to receive the report within 3-4 weeks.

ABA:Behavioral Consultation

Embodied Heart Space provides behavioral consultation services for parents/caregivers, other providers and/or school staff. It’s an opportunity to receive expert guidance and support as well as education about applied behavior analysis (ABA) principles and strategies. Dr. Michele, who is a board-certified behavior analyst (BCBA), will assess your child’s behaviors, strengths, and challenges (particularly for autism related concerns and other neurodevelopmental conditions), then helps you to understand evidence-based approaches that can be used at home, school or in daily life. Through this consultation process, you’ll learn practical techniques to support skill development and address challenging behaviors. This service is designed to empower you with knowledge so you can either implement strategies yourself in everyday situations or become a more informed advocate when selecting and working with an ABA therapist or program for your child. 

Dr. Michele does not provide direct ABA therapy with children. Rather, behavioral consultation is a collaborative partnership focused on equipping families, other caregivers and providers with the tools and understanding they need to best support their child.
